Please make sure to check the acknowledgement box with regard to having read and understood the rules.
When you are finished filling in the form, you can click on the "Send Form" button and Adobe Reader will step you through the process of e-mailing the data. (If you're viewing the form in another application, you can also save it and e-mail it as an attachment to rpoodiac@norwich.edu.)
You can also click the "Print Form" button to get a copy of the form for your records, fill out the form by hand, or to send via snail mail.
If something goes wrong along the way, either clicking the "Reset Form" button (or the "Reload" button on your browser) will allow you to start over.
